Clare is a Division 1 Registered Nurse with 34 years experience – much of which has been spent in major hospitals in the areas of Intensive Care, emergency departments, paediatrics and medical/surgical units.
Over the past 11 years, Clare has operated as a Practice Nurse working in the fields of Chronic Disease Management, Over 75 years Health Assessments and Advance Care Planning.
More recently, she’s been a Veterans’ Care Coordinator.
These experiences led Clare to see the need for AgeSmart and the unprecedented benefits of comprehensive care planning for older people wanting to remain independent in their own homes.
Clare brings a wealth of experience in identifying and accessing all the supports needed. She aims to allow clients to have control over their care by giving them options and working in collaboration with them to devise a tailored care plan that gives everyone total peace of mind – including families and carers.
Clare is also a qualified Advance Care Facilitator. This embraces planning for future care should a person become unable to make decisions for themselves due to progression of disease (for example, Alzheimer’s Disease). She can prepare documents (Medical Power of Attorney, Refusal of Treatment and Advance Care Directives) which describe the person’s wishes for their care by medical and nursing staff in the event they are hospitalised and unable to express their own preferences.
Clare has four grown-up children and enjoys acting in amateur theatre.
